We collect the following general data that is not personally identifiable information through 401(k) PRISM Report Card and/or 401(k) PRISM Analysis Report.  We collect demographic data in order to compare 401(k) PRISM Report Card vs. other 401(k) plan markets. 

We verify the accuracy of this information by a confirmation email.

We collect this information through secure online form, upload, and fax.  To receive our services, current 401(k) plan data is mandatory to compare among peer groups.

We do not employ cookies. A cookie is a small text file that a Web server places on a user’s computer hard drive to be a unique identifier. Cookies enable to track usage patterns and deliver customized content to users. 

Part II.  Information Usage.
The information collected by 401(k) PRISM, Inc. will be used for enhance 401kprism.com by relevant 401(k) material.  Users who provide information will have the options to subscribe 401(k) updates and newsletters.

Registered users, who opt in, will receive additional announcements of services and special deals. Out of respect for the privacy of our users, we present the option to not receive these types of communications.

The information we collect will not be used to create customer profiles based on browsing or purchasing history.  We will not supplement information, collected at our Web site, with data from other sources such as credit history to authorize purchases.
